Warum und wie starb der Taucher Knut Aksel ISAKSEN am Heilig Abend 2009

Es war ein Schock für die ganze Insel. Mitten in den fröhlichen Weihnachtsvorbereitungen während der Mittagszeit am Heilig Abend, 24 Dezember 2009, erreichte uns die traurige Nachricht über den plötzlichen Tod eines norwegischen Tauchers während eine technischen Tauchganges (Fun Tech Dive) bei der versunkenen Insel MONAD SHOAL, Malapascua.








Und schon wieder "MALAS PASCO" Böse Weihnachten auf "MALAPASCUA"? Ist "Heilig Abend" ein schlechtes Omen für Malapascua???

Ein tragischer Tauchunfall passierte am Heilig Abend (Dec.24 2009) in der Nähe von MONAD SHOAL/Malapascua Island. Die versunkene Insel Monad Shoal in der Nähe von Malapascua ist bekannt dafür dass man dort morgens in aller Frühe in der Morgendämmerung zwischen 5 und 6 Uhr auf Tauchgang gehen kann um dort die Tiefsee Fuchshaie (Treshersharks) beobachten zu können. In einer Tiefe von 20-25 Metern liegt diese "versunkene Insel" mit einer Grösse von annähernd 1000 meter * 500 meter und fällt von dort ab in die Tiefe der Visayan Sea bis zu 200 Meter. Tagsüber während der Mittagszeit kann man dort auch andere Grossfische beobachten, wie z.B. den Manta oder auch Hammerhaie.

Knut Aksel ISAKSEN (45) war ein gebürtiger Finne, lebte aber in Norwegen und betrieb selbst ein Tauchgeschäft. Er war PADI Tauchinstructor und ein äußerst erfahrener Taucher sowohl in den nordischen Gewässern als auch in den Tropen. Seine Tauchbegeisterung führte ihn unter anderem nach Florida, Ägypten Thailand und besonders in die Philippinen wo er immer wieder hinkam um weitere Taucherfahrungen zu sammeln und zusätzliche Tauchkenntnisse in speziellen Tauchkursen zu erlernen. Über die Weihnachtsfeiertage 2009 kam er mit seinem Tauchfreund Mr. Nikola Olsen erneut nach Malapascua. Beide buchten am 11 Dezember im Exotic Tauchshop ein und wollten bis zum 1 Januar 2010 dort bleiben.  Knut Aksel ISAKSEN wollte in Malapascua seinen Divemaster in TechDive absolvieren. 

Knut war äußerst beliebt, ein erfahrener Taucher, sportlich fit und immer gut gelaunt und zu Späßen aufgelegt. Neben seinem Taucherhobby pflegte er auch sein Motorradhobby und war öfters auf Touren mit seinem KAWASAKI Sporttourer.





An jenem schicksalhaften Tag dem 24.Dezember 2009 war noch einmal ein TECH DIVE Tauchgang bei MONAD SHOAL vorgesehen, bevor die Weihnachtsfeier im Resort losgehen sollte. Knut Aksel war gut gelaunt, das Wetter war toll, Sonnenschein, ruhiges Wasser, gute Sicht, also alles in allem ideale Tauchbedingungen. Auch die gesamte Stimmung im Resort, bei  den Freunden, Dive Master und Divebuddies war gut und man war fröhlicher Dinge wie bereits Tage zuvor.










Das Tauchboot verliess so zwischen 9 und 10 Uhr morgens bei strahlendem Sonnenschein die Tauchbasis Richtung Monad Shoal. Dort angekommen wurde die Tauchausrüstung angelegt und dann begannen die Tauchgänge in 2 Gruppen. Knut Aksel Isaksen ging mit seinem TechDive Divemaster auf Tiefe.


Gegen 11:20 Uhr (siehe Polizeibericht) auf dem Rückweg zur Meeresoberfläche bekam Knut Aksel erhebliche Probleme während einer seiner vorgesehenen Dekompressionstops und es begann das


DRAMA....


"...Durch Handzeichen alarmierte er seinen Dive buddy/Dive instructor welcher ihm dann zu Hilfe eilte. Aber unglücklicherweise starb Knut Aksel Isaksen in den Armen seines Diveinstructors..." 


Mit diesem letzten Satz wurde der tragische tödliche Tauchunfall vom Tauchveranstalter EXOTIC auf seiner Website eine Woche nach dem Unfall beschrieben. 


Aber was war der Grund für diesen tödlichen Unfall ? Der Police Report Aksel Isaksen gibt zur eigentlichen Unfallursache keine Erklärung. Zur Todesursache wird "Tod durch Ertrinken" testamentiert. Einer der in der Polizeiwache anwesenden Polizeibeamten wird mit den Worten zitiert "... eine genaue Untersuchung dieses tödlichen Unfalles war uns aufgrund unserer mangelnden Kenntnisse des Tauchsportes , deren Prozeduren, technischen Vorgaben, Bestimmungen und Vorschriften nicht möglich..." 


Bleibt für uns alle die Frage offen, wie konnte das passieren, was war die Ursache?



Im folgenden Link zu einer norwegischen Webseite erfahren wir -über die dürftigen Angaben des Tauchveranstalters hinaus-, wesentliche Details über den Unfallhergang bzw. über die eigentliche Unfallursache die etwas mehr Licht in die Geschichte bringen. War es der Tauchlehrer oder ein weiterer Taucher der diese Hinweise gab, wir wissen es nicht. Wie auch immer, nur Gott und der verunglückte Taucher wissen wie es wirklich war und keiner spricht mit uns.  Alles andere sind nur Hypothesen, Vermutungen, Entschuldigungen, Erzählungen, Geschichten:

          Ein 45 Jahre alter  Tauch Ausbilder aus Oslo wurde an Heilig Abend während eines  
         Technischen Spass Tauchens  im Urlaubsparadies Malapascua auf den Philippinen
         getötet. Ursache des Unfalles war daß er während eines Dekompressionsstops in 16 
         Meter Tiefe  eine zu große Konzentration an Sauerstoff in seiner Atemluft hatte. 
A 45 year-old diving instructor from Oslo were killed on Christmas Eve during a technical fun dive in holiday paradise of Malapascua in the Philippines. The accident cause was that he got to high concentration of oxygen in breathing air during a decompression stop at 16 meters.




DRAMA:


          Die Taucher hatten bereits bei einem Stop bei 21 metern ohne irgendwelche Probleme 
          durchgeführt bei welchem Knut seine Sauerstoffflasche auf 50% Sauerstoff wechseln
          wollte und dann auf die Sauerstoffflasche wechselte von der er dachte das es diejenige 
          mit 50% Sauerstoff sei. Er erkannte in diesem Moment nicht daß er den falschen 
          Sauerstoff hatte (100%). Die Taucher machten sich auf den Rückweg nach oben zum
          nächsten Dekostop. Bei ungefähr 16 meter während des DekoStops bemerkte der 
          Tauchausbilder das irgendwas schief lief bei Knut. Knut taumelte und verlor die 
          Orientierung, hatte Krämpfe und KEIN ATEMVENTIL IM MUND. Der Tauchausbilder 
          versuchte nun unverzüglich Atemluft in den Mund des Opfers zu bringen um dessen 
          Leben zu retten, aber ohne Erfolg,  er konnte den Kiefer des Opfers nicht öffnen welches
          durch Krämpfe blockiert war.
 The divers had already conducted a stop at 21 meters without any problems where Knut wants to change his oxygen bottle to 50% and switched to the gas bottle he thought that should be 50% oxygen. He did not realized at this moment that he got the wrong oxygen(100%). They went back to go up to the next deco stop. At about 16 meters during their  decrompression stop the dive instructor realized that something went wrong with Knut.. He was tumbling and lost orientation he had cramps and he had NO VALVE IN HIS MOUTH. So dive buddy tried immediatly to get air in the victims mouth to save his life, but without success cause he could not open the victims jaw which was locked in spasm. So Knutwent OUT OF AIR .


          Der Tauchausbilder tat alles was er tun konnte um das Leben des Opfers zu retten, 
          musste ihn aber letztendlich an die Oberfläche schicken um seine eigenen 
          Dekompressionsstops durchzuführen.
          Das 45 Jahre alte Opfer kam eigentlich aus Finnland, war aber ein renommierter 
          Ausbilder in der Region Oslo wo er einer Sporttauchergruppe angehörte.


The dive instructor (Dive Buddy)  did what he could to rescue the victim, but had to finally send him to the surface in order to conduct his own decompression stops. The 45-year-old victim  originally came from Finnmark, and was a renowned instructor in the Oslo area in which he was affiliated with Sport Diver.

Ursache falsches Gas
          Der Unfall dieses Tauchganges ist passiert weil zwei der stage bottles welche für die
          Dekompression benötigt wurden, VERTAUSCHT WAREN. Jeder der beiden Taucher
          sollte eigentlich jeweils 2 stage bottles mit sich führen, eine mit 50% Sauerstoff und eine

          andere mit 100% Sauerstoff.
          Als die Dinge anfingen schief zu laufen entdeckte der Tauchausbilder daß das Opfer
          falsche Sauerstoffflaschen mitführte, anstelle von einer mit 50% und einer mit 100%
          Sauerstoff -  enthielten beide Flaschen 100% Sauerstoff !



Error gas
The accident is under the diving experience has happened because two of stage bottles to be used for decompression stops were confused. The two divers should have with them each two stage bottles,  one with 50% oxygen and one with 100% oxygen.
When things went wrong the dive instructor  discovered that the victim had brought the wrong stage bottles, instead of one with 50 % and one with 100% oxygen - both stage bottles contained 100% oxygen !
The dive instructor controlled his own two bottles, both of which proved to contain 50% oxygen, instead of one with 50% oxygen and one with 100%. 



           Das Opfer hatte Krämpfe in 16 meter Tiefe, und hatte keine Atemventil im Mund.
           Der Tauchlehrer versuchte Atemluft in den Mund des Opfers zu bringen, aber der
           Kiefer des Opfers war durch Krämpfe gesperrt und konnte nicht geöffnet werden.
     
           Es scheint daher klar, was den Unfall verursachte - das Opfer atmete zu hohe
           Sauerstoffkonzentration, was Krämpfe und Atemstillstand zur Folge hatte. 


The victim had cramps at 16 meters depth, and had no valve in the mouth. dive instructor tried to get air into the mouth of 45-year-old, but the jaws were locked in spasm. It therefore seems clear what caused the accident - the victim was breathing too high oxygen concentration and had convulsions and respiratory arrest as a result.

Musste abgeschnitten werden

         Während der Versuche des Tauchausbilder das Leben des  Oslo-Mannes zu retten,
         verwickelten sich beide in die Leinen der Oberflächen Bojen. Der Tauchausbilder musste
         sie aus den Schnüren erst losschneiden, bevor er das Opfer an die Oberfläche schicken
         konnte. Das 45 - jährige Opfer wurde dann schnell from Tauchboot entdeckt, welche  die
         Oberflächenmarker (Taucherbojen) entdeckten welche die beiden Taucher hoch-
         geschickt hatten.



         Es wurde Reanimation versucht, aber das Leben des Opfers war nicht zu retten.

Einem engen Freund zur Folge, der als Sprecher der Familie fungiert, war das Opfer stets darum besorgt, dass Ursachen von Tauchunfällen ans Licht kommen und umfassend untersucht werden sollten.  Diesem zufolge ist das hiermit getan und die Tauchergemeinde hat ihre Information über diesen tragischen Unfall. Die nächsten Angehörigen der Familie wurden informiert.


Had to cut loose





While the dive instructor tried to save the life of the Oslo-man they have been wrapped the two into the lines from the  surface buoys. The dive instructor had to cut them loose before he could send the victim to the surface. The 45-year-old was quickly spotted by the dive boat, which had seen the surface markers, the two divers had been sent up.
Resuscitation was attempted, but life was not to save.
According to a close friend who acts as spokesman for the family, the victim was very concerned that the causes of diving accidents should come to light and get fully investigated.. It is accordingly that diving community has gotten their information about this tragic accident. The next members of  the family have been notified.

If I should die while diving.
If I should die while diving please do not hesitate to discuss the incident and assess every element with a view to furthering your understanding of how to enhance diver safety.





If I should die while diving get the facts. They won't be readily available and will definitely not be correct as reported by the media. But get the facts as best you can.
If I should die while diving understand, as I already do, that it will most likely involve fault on my part to some degree or another so do not hesitate to point that out.
If I should die while diving some of the fault will probably belong to my buddy and that needs to be honestly assessed as well though I must admit this is one area where I hope that compassion will be in the mix.
If I should die while diving there might be those who try to squelch discussion out of a misplaced notion of respect for the deceased, family and friends. They can say nice things about me at my funeral... but in the scuba community I want the incident to be strongly discussed.
I'd like to see dive operators offer a copy of this with their liability release.

But until now and after all there is a question, was there a possibility to save his life during this accident?

Speculation postulates possible scenarios in the absence of known facts. Often narrowing a multitude of possible circumstances down to a few high probability ones. While this will not provide a precise determination of cause of accident, (why did they had the wrong gas bottles?, could the victim stayed alive when the dive instructor have give him his own 50% gas bottle?)  the view afforded by speculative analysis into possible and probable causes, along with the insight on how they occur and how to prevent them - does serve the purpose of this article.

What difference does it make for you to know how I died?
I see the point. What difference does it make when a stranger dies if their passing does not have a direct significant impact on us? It is the possible lessons we can learn through an examination of a real life experience. There are useful lessons and reminders that may be necessary for many to keep complacency away and correct and improve errant and deficient ways, that may keep us away from a similar experience. Not all the accidents are fatal. The fatal ones do have a way to make a sometimes necessary poignant impression.
Given the information available, and the diversity and scale of participants knowledge, the reasonable speculative analysis that takes place here is very effective and productive in fulfiling the purpose of this article - which is to provide a learning avenue for some or for all, sometimes or always, through analysis of possible causes of accidents.

I have said this before and will continue to say it. There is enough difference of opinion on this subject to warrant the creation of a condolences forum designed to pay tribute to a lost life, devoid of undesireable issues that must by necessity be raised during accident analysis. This is the only way to accomodate these two seperate legitimate issues which both appear to be in high demand, that do not lend themselves to peaceful co-existence in a forum. Then there is the desire, by a not insignificant number, to restrict reasonable discussion because it violates their personal standards - not at my expense, unless a more convincing yet to be made case is presented.





"The golden rule is that there are no golden rules."






Something worth pointing out is that only the actual incident analysis is useful. The reports that are squelched "waiting for the facts to come out" and then swept under the rug don't help the next guy. The details that don't come out because people are afraid of getting sued definitely don't help the next guy. The condolence threads make everyone feel better but tend to be moderated "in case relatives might read them" absolutely positively don't help the next guy. We learn from mistakes, that is just part of being human. That can only happen if we learn the facts, or as much of the facts as we can.

... post pictures, video, interviews, autopsy report, documenation, dive logs ON the Internet. The more information you can provide the more lively the discussion and if it saves a life that would rock too.






Sure, we might just be beating a dead horse by reporting and analyzing every accident, but it's a horse that needs to be beaten.

Besides, you never know exactly what minor detail someone will take away from an incident report that will save their life or make them a better diver.

I have read alot concerning death while diving for educational purposes. I cant believe all of the "lets not speculate" and "have repect for the dead" crap I have read, like "....We have delayed posting this information out of respect for the family...
I wanna read about the rumors, speculation, facts and lies. I've always believed in the idioism "where there is smoke there is fire. With in the lies there is truth.

I have come to say that only the diver and God really know what happened and neither is talking to us.






I am not a diver, but is this a reason not to ask what happend? I want to know what happend and why, for the safety of my diving friends and family and diving guest what went wrong in an dive accident. 






I don't buy into censoring or quashing discussion because words might hurt someone. That is a very dangerous way to think about speech.




The reasoning behind analysing incidents is to find what went wrong. To learn why things happened the way they did, and to teach other to not make the same mistake. It is not to lay blame on who did what. The idea is to prevent others from falling onto the same set of circumstances and making the same fatal mistakes. Blaming the one who gives information to the diving community about an dive accident instead of search for the truth is the wrong way to protect the divers from the next deadly accident



Police report

SUBJECT:   Drown Person Alarm Involving Foreign National
FACTS:
        Initial Investigation conducted to Marcelino Rosales and Melchior Alejandre, both residents of
Malapascua Island, crew of EXOTIC 1 Pumpboat used for diving of some clients foreign national
disclosed that victim Knut Aksel Isaksen, male, a Norwegian National were on divingactivity at
Monad Shoal at around 11:30 A.M. to date December 24,2009 with three(3) other companies, which
are also foreign nationals named Xavier and Nicola, both male and instruc tor Tim Latimer, a Canadian
national. While diving, the two (2), Xavier and Nicola surfaced on the sea and minutes after, they saw
the victim Knut Aksel surfaced on the sea in a dorsal positionunconscious. Right there after they
rescued and rwvive the victim by administering oxygen. As they noticed that victim needs immediate
medical rescua, they went back to "Dutch Phil Dive Shop and Beach Resort(*1)" and right there the 
manager of said beach resort tried to rescue the the victim but despite of they finally decided to bring
and referred the victim to Daanbantayan District Hospital. Post mortem was conducted by Dr.
Nicomedes I.Jabel, of RHU, Daanbantayan, Cebu and the latter disclosed that caused of death is
indeed due to drowning. Alexander Palanas, Security Officer of said beach resort disclosed that
victim Knut Aksel arrived the island last December 11,2009 and checked in at said beach resort of
same date and suppose to check out on January 1, 2010, as also appeared of victims passport
Booked for record and for further investigation.
